• When a candidate comes into the office to complete a Notice of Candidacy, he should complete a Statement of Organization, Certification of Treasurer, and, if applicable, an Organizational Report. This will be completed even if the candidate has previously filed a Statement of Organization. If the candidate indicates he/she has previously completed a Statement of Organization, they will complete a new form and check the box that indicates that this is an Amendment. Every candidate should complete a Certification of Treasurer form, even if the candidate appoints himself as Treasurer.
• If the candidate wishes to file under the $3000 threshold, they will indicate this by checking the threshold box on the Statement of Organization. All other information on the Statement of Organization must be completed. If the candidate has filed under the threshold, an Organizational Report is not required. If the candidate did not file under the threshold, an Organizational Report must be filed.

Upon completion of the Statement of Organization, the county name, name of candidate, address, and office sought of the candidate will be emailed to the State Board of Elections. At the end of filing, candidate numbers will be assigned and the State Board of Elections will mail the information back to the counties for distribution to the candidates.
