Additional Services Breastfeeding Advisory Council
Pitt County Breastfeeding Advisory Council is an official council recognized by the Pitt County Board of Commissioners. The Advisory Council membership is made up of individuals and agencies that support and encourage breastfeeding as the optimal form of infant feeding. Community Care Plan of Eastern Carolina (CCPEC)
The Community Care Plan of Eastern Carolina is one of North Carolina's fifteen networks for managing the care of the Medicaid population. This public-private partnership builds on the primary care case management model called Carolina Access which was introduced in the early 1990's.
Under this model, enrollees choose a primary care provider who is responsible for coordinating the care of his or her enrollees. The CCPEC expands this model to include a network of providers and organizations that care for the Medicaid population in eastern North Carolina. The program, whose goals are to improve access, quality and costs, links the services of primary care providers with those provided at the local hospital, Health Department and Department of Social Services. Nurse Case Managers provide high-risk individuals with education, guidance and support. Current care management initiatives include asthma, high-risk obstetrics, diabetes and frequent utilizers of the hospital's emergency department. The Food Literacy Partners Program is a project of Nutrition
Partners (a subcommittee of Pitt Partners for Health). The Program is designed to train and certify community volunteers to provide nutrition and physical activity education in a variety of settings. An estimated 20,000+ people in Pitt County are uninsured and do not have access to a primary care physician. HealthAssist is a partnership between local communities and organizations including Pitt County Memorial Hospital, Pitt County Medical Society, private physicians, Pitt County Health Department and ECU School of Medicine to provide access to medical services including prescription medicines, social services, health education and other programs. Community improvement partnership with representatives from local churches, businesses, communities and human service agencies. The partnership responds collaboratively to the compelling health needs of Pitt County residents through assessment, resource identification, development of citizen advocacy, comprehensive planning and coordination of health intervention and prevention strategies. Presently, the five priorities for Pitt Partners for Health include Access to Care, Heart Disease and Stroke, Diabetes, Nutrition and Physical Activity, Older Adult Health, and Substance Abuse. The Martin-Pitt Partnership for Children, Inc. (MPPFC) is the Local Smart Start Agency for Martin and Pitt Counties. Crossing boundaries, the Martin-Pitt Partnership for Children, Inc. will work together through community and family efforts to ensure that all children between birth and 5 are able to enter school healthy and ready to learn. Winner’s Circle is a statewide program with three objectives: 1) to create healthy eating environments in North Carolina, 2) to give consumers consistent, sound and easily recognized nutritional guidance in participating restaurants, 3) to increase consumer demand for healthy restaurant items.
