131239	3/9/2007	On the voucher screen in Success.net that once a voucher has a termination date in the termination field it becomes read only and once a provider is saved in the provider field it becomes read only.  Note:  If Facility name is "Update Facility Name", then the facility name can be changed.  This allows for a care plan to be set up if the Facility has not been entered into SuccessNet yet using "Update Facility Name" and changing the facility name once entered into the system.	Updated voucher screen to ensure facility cannot be changed after saving.  (Note:  If Facility Name is "Update Facility Name", then the facility can be revised.  This allows for a voucher to be entered prior to having the information for the facility entered in to SuccessNet.)  Ensured that voucher could not be edited if termination date not null.  Updated action notices appropriately.	1.3.0.1
131239	3/9/2007	On the voucher screen, fix problem with more action notice being put into history for more than one care plan.  For instance, if one care plan was changed for start date and another care plan was changed for voucher returned, an action notice would appear for both care plans pertaining to the start date action.  This would only occur if more than one care plan was updated on the voucher screen.	Ensured that flags for changes were reset after each care plan processed.	1.3.0.1
