What information will be needed to process my Food Stamp or Medicaid application?
You will need the following items:
1. ID
2. 2 forms of residence if applying for Medicaid (one form for Food Stamps)
3. All income received in the prior month of application for all those applying. (example applying in May, will need all income for April)
4. If applying for Medicaid for a child - tax filing unit (who claimed the child on the taxes)
