Even if you are unconscious or unable to speak, this tool ensures your information is available when needed.
Whether you are parents of a new family, a single adult living alone, or an older adult managing multiple medications, the Vial of Life can provide peace of mind. By completing a simple form and storing it in a designated container, you make sure emergency personnel have the details they need when time is critical. Information can include:
- Name, date of birth, and photo (optional)
- Current medications and dosages
- Blood type, allergies and medical conditions
- Emergency contact information
- Primary healthcare provider details
How the Program Works
- Fill and provided important medical information.
- If more than one person in the household uses the Vial of Life, complete the information for each person and place them together in the container.
- Place the information inside a container such as a pill bottle marked Vial of Life.
- Store the container in an easy-to-find location, such as your refrigerator or freezer.
- Place A Vial of Life sticker for your front door
